部份中文替罗非班处方资料(仅从参考) 商品名:AGGRASTAT 英文名:Tirofiban 中文名:替罗非班浓缩输注液 藥理類別:血小板凝集抑制劑 孕婦用藥分級 B級: 在对照的动物生殖研究试验中未显示该药物有胚胎毒性,但尚无完整的人体怀孕妇女研究试验资料;或者在动物生殖对照研究试验中发现该药物有不良反应(大于降低生育力之反应) ,但在对照的人体怀孕妇女研究试验中,未显示该药物有胚胎毒性。 结构式 Image:Tirofiban.svg N- (butylsulfonyl)-O-[4-(4-piperidinyl)butyl]-L-tyrosine UpToDate UpToDate 連結 药理作用 GPⅡb/Ⅲa是血小板上的一种receptor;而一般血块的形成是当血管受损后,血小板会与受损血管的内皮下层所曝露出来的胶原黏附(adhesion),进一步刺激一些化学物质像是ADP、thrombin、epinephrine等的释放,而这些化学物质会活化血小板上的GPⅡb/Ⅲa receptor,此外,在血小板凝集的最后步骤,纤维蛋白原(fibrinogen)会经由VWF(von willebrand factor)之修饰而改变形状,变得更易与GPⅡb/Ⅲa receptor结合,而使得血小板和血小板之间产生交叉连结。GPⅡb/Ⅲa receptor antagonist(血小板糖蛋白Ⅱb/Ⅲa接受体拮抗剂),则是藉由阻断纤维蛋白原(fibrinogen)和糖蛋白Ⅱb/Ⅲa接受体(GPⅡb/Ⅲa receptor)之结合,而达到抑制血小板凝集之作用 适应症 AGGRASTAT并用HEPARIN,可用于预防不稳定心绞痛病人或非Q波之心肌梗塞病人发生缺血性心脏病发作,及用于预防患有冠状动脉缺血症状而须接受冠状动脉成形术或冠状动脉粥状硬化切除的病人发生治疗冠状动脉突然闭塞时所造成的缺血性心脏并发症。 用法用量 AGGRASTAT须采静脉注射,并与heparin并用,起始输注速率为0.4 microgram/kg/min,输注30分钟。完成起始输注后,应再继续以0.1 microgram/kg/min之维持输注速率持续输注。 药动力学 分配 Vd范围为22至42L。不与血浆蛋白高度结合(35%存在于血浆中作为未结合的药物)。 消除 尿液排出65%,粪便排出约25%,主要是未改变的药物。 t½约为2小时。 特殊人群 肾功能损害 CrCl低于30mL/min的患者血浆Cl显着降低(超过50%)。 肝功能损害 轻度至中度肝功能损害的血浆Cl与健康患者无显着差异。 老年 血浆Cl低约19%至26%。 副作用 出血的危險性;噁心、發燒,頭痛。 互相作用 阿司匹林,肝素 出血风险增加。 影响止血的药物(如华法林) 谨慎使用,可能会增加出血风险。 左旋甲状腺素,奥美拉唑 替罗非班清除率可能会增加; 然而,临床重要性尚不清楚。 禁忌 1.对本药中任一成份过敏之病患应禁用aggrastat。 2.有下列临床症状者应禁用:进行性出血;曾有颅内出血、颅内肿瘤、动静脉血管畸形或动脉瘤者;先前曾注射aggrastat而引发血小板减少症者。 给药规定 2.1.1.3.Tirofiban (如Aggrastat): 1.限不稳定型心绞痛,对传统疗法无反应者,已预定施行经冠状动脉成形术之不稳定狭心症时,作为heparin的辅助疗法,或非Q波之心肌梗塞之病患使用。 2.急性Q波心肌梗塞于症状发生十二小时内,于执行PTCA时得并用tirofiban。 注意事项 最近一年内出血,已知的凝血病变,血小板异常或血小板缺乏症病史;血小板数目小于150000细胞数∕立方毫米。最近一年内有脑血管疾病者,最近一个月内大手术或严重身体外伤。主动脉剥离的病史;严重控制不良的高血压;急性心包膜炎;出血性视网膜病变;经动脉放置骨动脉外鞘;严重肾功能不足。 药品保存方式 药品应置于摄氏15 ~ 25度干燥处所,不可冷冻,避免光线照射;如发生变质或过期,不可再使用。 英文说明书: Brand Name Aggrastat Common Name tirofiban How does this medication work? What will it do for me? Tirofiban belongs to the family of medications known as platelet aggregation inhibitors. Platelets are a component of the blood that is very important to blood clotting. Heart attacks are caused by blood clots that block the required flow of blood to the heart. Tirofiban works by preventing platelets from forming blood clots. It is used in combination with heparin (a medication also used to prevent blood clots) and acetylsalicylic acid (ASA) to prevent additional heart attacks or unstable angina (chest pain that gets worse with time and puts a person at risk for heart attack) for people who have recently experienced such events. This medication may be available under multiple brand names and/or in several different forms. Any specific brand name of this medication may not be available in all of the forms or approved for all of the conditions discussed here. As well, some forms of this medication may not be used for all of the conditions discussed here. Your doctor may have suggested this medication for conditions other than those listed in these drug information articles. If you have not discussed this with your doctor or are not sure why you are taking this medication, speak to your doctor. Do not stop taking this medication without consulting your doctor. How should I use this medication? Tirofiban is used in a hospital setting and is given by injection. A needle is inserted into a vein in the arm and the medication is injected slowly over a period of time. The medication is usually administered for a period of at least 48 hours for people who have had a heart attack or who have unstable angina. The dose of medication required is based on a person's condition and body weight. Many things can affect the dose of medication that a person needs, such as body weight, other medical conditions, and other medications. If your doctor has recommended a dose different from the ones listed here, do not change the way that you are taking the medication without consulting your doctor. It is important to use this medication exactly as prescribed by your doctor. Store this medication at room temperature, do not let it freeze, and protect it from light. Keep it out of the reach of children. Do not dispose of medications in wastewater (e.g. down the sink or in the toilet) or in household garbage. Ask your pharmacist how to dispose of medications that are no longer needed or have expired. What form(s) does this medication come in? Solution for Infusion Each mL of solution for infusion (premixed, iso-osmotic solution) contains tirofiban free base 50 µg (as tirofiban HCl). Nonmedicinal ingredients: citric acid anhydrous, sodium chloride, and sodium citrate dihydrate. The pH ranges from 5.5 to 6.5 and may have been adjusted with hydrochloric acid and/or sodium hydroxide.
